Hello, Maryland Chapter members. Michelle Moccia (Past President of GAPNA) and current GAPNA Chair is inviting you to hear about a new service to improve care for older adults with a hospital partner in Maryland. 

ApogeeCare is a telemedicine company on a mission to improve patient care for older adults. Our geriatricians provide comprehensive, compassionate care to frail older adults in the emergency department (ED). Working collaboratively with ED staff, we develop tailored 4M age-friendly care plans that prevent the hazards of hospitalization and allow patients to return home safely. With those goals in mind, we are hiring!

ApogeeCare clinicians perform comprehensive geriatric assessments for older adults in the emergency departments (ED) via telehealth. In early 2025, we will launch this innovative care model with a hospital partner in Maryland. ApogeeCare addresses a substantial clinical need and will soon expand to other states.
